Purchase Order

We accept purchase orders from qualified corporations and organizations. Please submit the purchase order to Emetrix by fax ( 801-497-9456 ) or email ( sales@emetrix.com ). Please include complete information including the product you wish to purchase, your billing address, phone number, etc. You MUST include an email address for product delivery.
